If you cannot attend your appointments for any reason, please let us know within 1 working day of your appointment, or as soon as possible, so that we can offer the appointment to someone else.

• are registered at the surgery

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
